Locum placement and contact map
Applicant playbookExplain locum sites and your documented employer
Keep the workplace, the document-named employer and the person confirming a fact distinct.

Short answer
Use the employer or agency named in your own records, then list the completed host placements with their actual dates and source records. Identify separately what the employer contact and each host contact can confirm. Do not infer your employment status from the word locum or count successive host sites as simultaneous jobs.

Practical example
Filled example: one named employer, two completed sites
This arrangement is entirely fictional. Sam, the organisations, records and contact replies are invented. The record expressly names the employer; the example does not classify real locum workers.
- Employment record | Example HealthStaff record dated 30 September 2026 | Names Example HealthStaff as employer, Sam’s role as nurse and original start date as 5 May 2025.
- Completed placement A | Example North Clinic, 2 to 13 September 2026 | Host completion note dated 13 September confirms those site dates.
- Completed placement B | Example Valley Clinic, 16 to 27 September 2026 | Host completion note dated 27 September confirms those site dates.
- Contact scope | The agency contact confirms it can address the named employment record | Each host contact confirms it can address its own placement dates | Sam does not ask a host to confirm agency-wide employment or pay.
- Future booking | A provisional placement for 19 to 30 October appears in a separate booking note | It is labelled provisional, not completed work or payment already received.
Working checklist
Use one placement strip and one contact card
Record completed placements separately. A contact name is useful only when you know which fact that person can address and have followed the relevant contact instructions.
- Employer or agency: the exact document-named organisation, role and source date.
- Placement: host site, completed start and end dates, and the record supporting those dates.
- Employer contact: the specific employment or engagement facts that contact has confirmed it can address.
- Host contact: the site attendance or placement facts that contact has confirmed it can address.
- Future entry: keep a provisional or future placement outside completed work and received-payment history.
Action plan
Copy and adapt: clarify the employer and contact
Send a narrow question to the issuer when the names or contact roles are unclear. Do not include patient information or clinical case details.
Subject: Clarifying my locum records for a rental application Hello [issuer contact], My [record title] dated [date] names [organisation], while the completed placement records list [host A and dates] and [host B and dates]. Could you confirm which organisation the record identifies as my employer or engagement counterparty, and which existing record explains that relationship? Please also clarify which contact can address the employment details and which can address the completed site dates. I will check the recipient’s contact instructions before passing on those details. Thank you, [name]
Use the answer only for the facts it addresses. Confirmation of site attendance does not, by itself, settle another organisation’s employment or payment details.
Useful context
Begin with the arrangement in your records
Read the employer or agency name in the relevant employment or engagement record before describing the host sites. If different documents use different names, keep the discrepancy open and ask the issuer to clarify it.
The example below explicitly uses one fictional agency employer. Your arrangement may differ. This worksheet does not decide employment classification, calculate income or establish a required locum document list.
Keep in view
Keep the explanation limited to the request
Provide information relevant to the recipient's application process and handle identity or financial material carefully.
Check the completed dates and contact scope against the selected records. Keep patient details, unrelated clinical information and unsupported future work outside the explanation. If an interstate appointment also needs clarification, handle its time separately.

Questions
Common questions
Clear answers for the decisions people usually pause on.
Should I list every host site as a separate employer?
Use the relationship actually described in your records. In this fictional example the agency is the named employer and the sites are placements. Do not assume that arrangement applies to you.
Can a future placement count as completed work?
Keep it separate and use its actual status, such as provisional or confirmed future booking. Neither label says the work is completed or a payment has been received.
